Thank you for registering for mediation services and applying for Reduced Fees. Once we receive your application and review your supporting documents, then we will contact you about your fees.

Only upload documents if you are applying for Reduced Fees.

You must submit one of the following documents supporting your annual gross income:

  1. First page of previous years U.S. Individual Income Tax Return (IRS 1040) showing annual income.

  2. Two most recent paycheck stubs from your place of employment (or a written statement from employer verifying your income).

When you are ready, you may upload documents, pictures, or files from your computer or phone on this page.
